Hack the Hood Case Manager Job Description
Location: Headquartered in OAKLAND, CA
Job Category: Full Time, non-exempt Employee
Reports To: Oakland Program Director
Compensation commensurate with experience

Hack the Hood was launched just five years ago to build a tech sector that works for everyone. We support under-resourced youth of color in communities around the state and the country to build 21st century careers in technology and social media. We turn the digital economy from threat to opportunity by joining our youth with the 58% of small businesses that don’t have a website. As one of the first organizations in this field, our programs aim to ensure that tech is making the world better for all the communities it intersects.

We are seeking a Case Manager who will evolve and strengthen our baseline wrap-around services. As Case Manager, you will interface with our programs team to better engage participants of our six to nine week boot camps and year-round membership programs. You will be central in providing case management support to under-resourced youth of color enrolled in our programs.  As Case Manager, you will assess, problem solve, coach, provide support and referrals for youth experiencing barriers to education, which have included: housing, transportation, wardrobe, income supports/benefits, child-care, health/mental health services, etc. As care-provider for our participants, you will be a critical part of developing strong referral relationships with service providers across Oakland and Alameda County.

RESPONSIBILITIES:
  • Lead the iteration of a new model that is better aligned with our core Community Cultural Wealth framework.
  • Meet individually with youth to assess their particular needs, develop a service plan, and support youth as they take steps towards their goals.
  • Provide links, referrals, and application assistance to social services and other supports that create the health and stability that enable positive education outcomes.
  • Establish and grow relationships with community partners to provide these services.
  • Maintain accurate and up-to-date records for each case, to support comprehensive reporting and documentation. This includes (but is not limited to): attendance, case notes, and grant reporting
  • Interact with youth as a resource and coach
  • Serve as a resource to staff in the areas of trauma-informed services and practices
  • Collaborate with staff on various projects related to refining and expanding reach of mental health services to our participants


QUALIFICATIONS
  • Degree in social work, LCSW preferred.
  • Experience in a social services or workforce setting providing case management
  • Knowledge and experience working with youth in under-resourced communities of color
  • Experience building and maintaining relationships and partnerships with community organizations
  • Experience with public speaking and training/workshop facilitation.
  • Excellent management skills, attention to detail, and ability to multi-task.
  • Ability to thrive and provide leadership in cross-cultural and cross-age contexts
  • Proficient in Microsoft Office, Salesforce, Google Suite
  • Alignment with Hack the Hood's mission and values
  • Strong personal integrity, maturity, credibility
  • Ability to give and receive constructive feedback

Desirable
  • Bilingual (Spanish, Vietnamese, or Mandarin/Cantonese)
  • Knowledge of and experience with HTH programs
